Cancellation procedures:

1. Contact the Advanced Protection Plan Administrator at 800.856.6468 and obtain a refund quote.
   
2. Complete the cancellation form and obtain the customer's signature.
   
3. Fax the form and waiver to 636.600.4426. All cancellations processed within 24 hours.
   
4. You may also mail in the form or you may include it with the Remittance Register
   and deduct the refund from the remittance amount.
